Helping Hawaiʻi Island Residents Talk About End-Of-Life Care
Having conversations about our end-of-life care choices can feel tough, but these conversations are vital to help ensure our healthcare choices will be respected and honored in all situations. Our Advance Health Care Directive initiative helps Hawaiʻi Island residents talk about their healthcare choices and create a legal document that gives them agency over their healthcare wishes.
Since 2015, we have helped more than 3,000 people complete Advance Health Care Directives. We offer free workshops to help people complete their Advance Health Care Directives and learn which healthcare choices they can make in advance.www.communityfirsthawaii.org/advance-healthcare-directive
